SUPPORTING OLDER ADULTS WITH RESOURCES PROGRAM (SOAR)

The Supporting Older Adults with Resources (SOAR) program helps older Marylanders remain safe, healthy, and independent in their homes and communities. SOAR provides case management, home-based support, caregiver assistance, and limited financial assistance for eligible seniors.

Services Coordination
•    Screening and eligibility determination 
•    In-home assessments 
•    Case management and care planning 
•    Assistance accessing community resources and benefits 
•    Gap-filling services to support independent living 

Individuals are screened to determine eligibility based on:
•    Age 62+
•    Income and assets 
•    Functional abilities and care needs 

Possible Services

• Case Management 

• Personal care assistance 

• Limited chore service 

• Medication costs 

• Medical supplies 

• Home-delivered meals 

• Transportation

• Adult day care 

• Emergency response systems 

• Partial financial assistance for assisted living fees
