Connecting your client or patient who has a chronic condition, to a self-management class near them, assists your practice and your patient. Your patient can receive support from an evidence-based curriculum geared toward building self-efficacy, problem solving and self-management skills. These workshops support and enhance medical interventions that you are currently using. As a result, your patients understand the importance of setting and achieving self-management goals as a part of their total care plan.

Refer your patients to one of our programs. These workshops will provide the skills and knowledge to better manage their chronic conditions, diabetes, chronic pain, hypertension, or depression. With their permission, a 3-month action plan and goal will be shared with you at the end of the workshop series.
