Do you need assistance with navigating your child's needs?

The Local Care Team (LCT) can offer support and connect your family with valuable resources in our community!  Multiple Carroll County organizations sit on the LCT and work with families to open doors, brainstorm ideas, share resources, and develop strategies to help children and families thrive.

The LCT can help your family if:

• A child or children in the household are experiencing multiple ongoing challenges such as escalating behavioral concerns, difficulties at school, conflicts with peers or family members, substance use, or barriers to accessing services; and
• The family is willing to attend and actively participate in LCT meetings and in developing a Family Action Plan.

Referral to the Local Care Team:

Families interested in the LCT can either be referred by a participating agency or refer themselves. 

• The LCT Referral Packet must be completed and sent to the LCT Coordinator (localcareteam@carrollcountymd.gov) for their review before a meeting can be scheduled.  
• For family self-referrals, please contact the Local Care Team Coordinator at 410-386-3600 for assistance with the referral process.

The Local Care Team meets virtually via the GoToMeeting platform.  In-person meetings can be arranged upon request and with advance notice.
